Reports to the Director of Professional Services. Provides and directs all aspects of each Hospice Home Health patient’s care. Assures quality patient care through professional assessment, interventions, evaluation and the use of the nursing process and nursing care plan.
Adheres to agency’s policies and procedures.
Maintains and manages a case load of 25 - 30 patients.
Plans, coordinates, and evaluates all aspects of care for Hospice Home Health patients in assigned territory, using the nursing process.
Provides skilled, preventative and rehabilitative nursing procedures as appropriate for the patient's safety,as ordered by the physician.
Communicates regularly with others to ensure patient's needs are met.
Communicates plan of care to the Hospice Supervisor and others on the health care team.
Keeps the Hospice Supervisor aware of all findings at initial and follow-up visits thru daily and PRN communications.
Communicates as needed with the patient's physician(s).
Supervises, teaches and evaluates others as needed.
Teaches, supervises and counsels patients and family members regarding nursing care, needs and other related problems of the patient at home.
Supervises Hospice Home Health Aide visits, and documents accordingly every two weeks.
Uses ancillary services in a timely and appropriate manner.
Maintains schedules and paperwork.
Maintains a daily schedule for all patients being managed and coordinates visits with the Hospice Supervisor.
Maintains clinical records on all patients and completes them in an appropriate and timely manner.

Attends and participates in meetings and in-services.
Participates in the On-Call rotation as needed and includes weekends and designated holidays.
Performs other duties as assigned.
Degree in nursing from a NLN accredited program.
Minimum of one (1) year experience as a professional nurse.
Current RN nursing license with the State of Employment
**Bi-lingual in Spanish a plus
Ability to resolve problems, set priorities, and function independently.
Excellent written and verbal skills with demonstrated ability to deal tactfully with patients and staff.
Successful completion of:
1. Negative TB test or Chest X-ray six (6) months prior to employment or within 30 days of the Date of employment and annually thereafter.
2. Satisfactory references.
Successful completion of orientation process.
Working knowledge and understanding of the functional organizational make-up of the Hospice Home Health Agency.
Competency in general nursing theory, techniques, medication, and practice, plus common knowledge of diagnosis, including signs and symptoms.
Ability to assess, intervene, and evaluate Hospice Home Health care provided to individual patients.
Knowledge and understanding of Community resources available.
Competency in I.V. infusion techniques and guidelines.
