The Social Services Coordinator is responsible for promoting desired patient outcomes by means of identifying emotional, social, or environmental barriers and using critical thinking skills to intervene accordingly. Responsible for providing services that address physical, emotional, economic, and social needs that have been identified through individual patient assessment. Is a member of the interdisciplinary health care team and actively participates in the development of the plan of care and assists in coordinating/facilitating care elements that involve psychosocial issues identified in assessment. Provides direct counseling services and provides education and referrals for patients and families to various resources and community agencies.
Position Minimum Requirements
Bachelor's Degree required in Mental Health Counseling, Social work, Psychology, or related field from an accredited college or university recognized by the Department of Education. Master's Degree and licensed-eligible in the State of Florida preferred. Basic Life Support (BLS) Certification required. 1-2years experience in hospital, healthcare, or behavioral setting required (may include internships).
