**Team 3-Central Indianapolis , Northwest Indy, and Westward to Danville **
DUTIES: Responsible for total case management of a group of home health care clients, providing direct nursing care, and coordinating and supervising other disciplines involved to promote positive health outcomes and quality services to patients. Responsible for providing skilled care to assigned patients according to the plan of care established by the patient's physician. Practices within the scope of nursing as defined by the Nurse Practice Act.
EXPERIENCE: One year nursing experience required. Home Care/Hospice experience preferred
LICENSURE: Current license to practice Registered Nursing in the state of Indiana & CPR certified. Must be a graduate of a NLN or CCNE accredited program.
ADDITIONAL INFORMATION: Compassionate, professional caregiver with excellent organizational, communication and critical thinking skills Nursing Degree required, BSN preferred.
