Plans and coordinates activities related to hospice patient and family needs at end-of-life. Works as an interdisplinary hospice team member providing the psychosocial emotional support. Collaboratively develops and implements a wholistic plan of care and makes regular patient visits and phone calls in assisting patients/families with needs. Manages the hospice family bereavement services and patient and family care conferences.

Provides social work visits as ordered and included in the home care patient's plan of care.


Possess thorough understanding of end of life care and dynamics at the end of life. Exhibits the ability to establish good working relationships with patients, families, staff, and other providers. Graduate of an accredited school of Social Work with a Masters Degree in Social Work (MSW). Current South Dakota licensure to practice as a Certified Social Worker (CSW). Two years experience in social work is preferred. Hospice social work experience is strongly desired.

Physical Demands

Must have access to reliable transportation, valid Driver's license and Motor Vehicle Liability Insurance. Work involves walking, stooping, reaching, climbing stairs, typing, grasping, feeling, talking, seeing, haring (no more than 40 % loss with/without correction). Requires occasional lifting, no more than ten pounds. Have basic computer skills with knowledge of email and Micorsoft Office programs.

Work Hours

Work hours Monday through Friday with occassional patient/family visits required on weekends or after office hours.
