The primary responsibilities are to provide community case management services to patients and families receiving home and community based long term care services. These case management services are to assist patients to remain at home with an array of supportive services for patients, families, and caregivers. This position assists in the timely resolution of patient and family problems by completing assessments including, establishing goals, and following through with referral to community services as needed. These case management issues may include issues related to paraprofessional services, legal, financial, spiritual, emotional, and/or interpersonal concerns. This position provides supervision and evaluation of both client care and personnel provided by agency PCAs and homemakers. The case worker is a liaison with other VNAVNH service providers in advocating for coordinated patient services in accordance with individually authorized State service plans. The case worker acts in accordance with VNA and Choices for Care (CFC) Case Management Certification policies and procedures. Intense training and orientation is provided for this home visiting position.

Bachelor's degree in Human Services, Social Work or RN required. Current Vermont and New Hampshire RN license required. Two year experience preferred working as a social worker or RN within a community setting. Knowledge and / or experience with community long term care resources or programs such as Choices for Care or Choices for Independence desired.
