needs.
The Health Records Representative provides patient
information upon request and assists patients with
birth/death certificates and submission of applicable
reports/information the Hawai‘i State Department of
Health. Complies with applicable policies and statutes
to safeguard the confidentiality of patient information.

Minimum Qualifications: High School or equivalent. One
year clerical experience in a position with customer
contact.
Preferred: Associate's Degree. Health records
experience.
