Job Description: Develop clinical and professional skills that will be used to provide comprehensive pharmaceutical services in a patient-focused health care setting.

Act as a resource for information and guidance to hospital personnel and physicians in medication therapy and management. Participate in formulary management and medication use evaluations as required by Therapeutics committee and specific rotations. Minimum Requirements: Must be a graduate from an accredited College of Pharmacy with a Doctorate of Pharmacy degree. Influenza vaccination will be required for all personnel working within LVHN, who have patient contact, unless medically contraindicated or due to a bonafide religious belief or practice.

Licensure and Certifications: Licensure as required by the Pennsylvania State Board of Pharmacy.
