Responsible for day to day physical therapy evaluation and treatment of customer needs. Is a role model of customer service principles and consistently monitors and implements quality efforts. Ensure that the department functions accordingly to accepted regulatory and quality standards.
ESSENTIAL DUTIES AND RESPONSIBILITIES:

Work with the interdisciplinary team to achieve/maintain the highest functional level of all customers and promote coordinated, efficient care.
Evaluate, develop and carry out physical therapy treatment plans in consultation with physician and in compliance with physician order. Monitor plans and modify when necessary.
Completeappropriate screening of customers in order to determine whether the individual warrants further, in-depth evaluation/assessment.
Performs therapy duties in all settings RG, OP and CHH as needed.
Demonstrate respect for, adherence to and promotion of the mission and philosophy of the Erickson Retirement Communities.
Ensure customer independence and right to make personal choices to accommodate their needs by informing and including customers and/or families in their plan of care.
Maintain customer confidence and protect facility operations by keeping information confidential in compliance with current community HIPAA guidelines.
Enforce patient rights regarding privacy, personal property and grievances.
Maintain and enforce current company policies and procedures and state and federal guidelines governing care delivery and documentation of physical therapy services.
Adhere to American Physical Therapy Association¿s (APTA) Code of Ethics.
Direct treatments provided by assistants, aides or students in accordance with state and federal guidelines.
Participate in care planning and discharge planning, attending meetings as indicated. Assure continuation of therapeutic plan following discharge from therapy by designing individualized maintenance programs, instructing customers, families, and caregivers and recommending adaptive equipment or follow-up programs.
Document services in medical and department records according to accepted company, regulatory and professional guidelines. Documentation must be accurate, concise, and timely, meet governmental and third party requirements, and reflect a clear picture of the problem, interventions and outcome.
Communicate significant changes in customer conditions to appropriate medical staff and interdisciplinary team members.
Provide and participate in educational and orientation programs.
Participate in facility and department meetings as appropriate.
Perform all duties in a safe and efficient manner and report unsafe conditions, complaints or incidents according to standard procedure to maintain a safe environment for customers, families, and staff.
Adhere to strict infection control guidelines as evidenced by appropriate use of personal protective equipment.
Demonstrate an awareness of fiscal management in service delivery and supply/equipment requests.
Respond consistently to the needs and concerns of customers, families, staff and visitors.
Communicate with the Rehabilitation Manager and Asst. Rehab Manager regarding department programs, goals, objectives, problems, and successes.
Audit documentation and other indicators for quality assurance.
Assist with the cleaning and maintenance of the treatment area and the rehabilitation department.
Maintain compliance with company productivity standards as appropriate.

EDUCATION and/or EXPERIENCE:
- Minimum of a Bachelors degree in physical therapy required. Degree and training must meet current Medicare personnel requirements for Physical Therapy.
- Need to utilize good reasoning and judgment skills to develop and adjust plans of care base upon status changes of the customer.
- Must be able to accurately complete documentation, billing, timesheets and tally evaluative tests.
- Experience in geriatric care across multiple diagnostic groups is preferred.
Supervision of Physical Therapy Assistants required in accordance with company, state and federal requirements. Intermittent supervision of Rehabilitation Technicians may be required as requested by the Rehabilitation Manager.
Must be able to document (both hand-written and via typed electronic documentation) clearly to meet reimbursement and regulatory requirements. Reasonable typing skills required. Must be able to speak clearly, be understood by customers and hear conversational speech.
LICENSES, CERTIFICATES, REGISTRATIONS:
Possess a valid current state license (for state in which the community provides service) in Physical Therapy, where applicable. Must meet CEU requirements to maintain valid state licensure in the state in which the community provides service. Current CPR Certification
