The Licensed Independent Clinical Social Worker (LICSW) provides direct services in the Transition Home Plus (THP) Program for high-risk preterm infants and their families focusing on positive outcomes and reduced re-hospitalizations. The Clinical Social Worker (LICSW) will work with the Medical Home Care Coordinator, to identify potential enrollees and approach parents/caretakers at an appropriate time to discuss involvement in the THP program. Together with the Coordinator, the incumbent will work with the NICU Medical Team, Social Services, RN Case Managers, and parents as discharge is planned. The incumbent will develop relationships with parents in the NICU that will continue after discharge to home. Pre-discharge home visits will be done as indicated to help parents assess their readiness for infant’s discharge to home. Post-discharge as a member of the THP team in the Neonatal Follow-Up Clinic, the LICSW will assess parents’ and infants’ needs for referrals to mental health, social and child development agencies, and overall community resources. Assists in project presentations and assists the Care Coordinator and physician as needed.

Master’s degree in social work, Licensed Independent Clinical Social Worker (LICSW) license required. Experience in a health care setting, the NICU, care coordination, or with families of high risk infants preferred. Experience with therapeutic, brief treatment and intervention techniques preferred. Proficiency of the English language required, bilingual in Spanish preferred. Excellent oral and written communication skills, organization, interpersonal, and presentation skills essential. Must have valid driver’s license and access to an appropriately insured automobile.
