The primary purpose of this position is to follow oral and written instructions, and perform routine procedures

according to established policies and care plans.

Responsible for the delivery of care, for a specific number of residents, that meets the standard of care

as directed per nurse aide assignment and resident care plans.

* Under close supervision of the Team Leader, following oral and written instructions performs routine procedures according to established policies and care plans.

* Under the guidance of RN, is responsible for the delivery of care, for primary group of residents, which meets the standard of care as per nurse aide assignment and resident care plans.

* Responsible for bathing, grooming and toileting needs of residents.

* Performs rounds every two hours for toileting and change of position.

* Provides assistance with Resident transfer, ambulation, positioning and lifting.

* Provides range of motion as required.

* Serves dietary trays, nourishments and fluids, feeds residents and supervises and encourages residents who feed themselves, completes meal intake board, I & O sheets, and nourishment sheets.

* Answers residents call lights within prescribed time frames.

* Takes care of Resident's clothing, checks for nametags, sorts and places soiled clothing in proper receptacle.

* Changes linen weekly and as necessary.

* Reports any change in Resident's condition or behavior.

* Reports incidents/accidents to Team Leader.

* Completes required paperwork.

* Collects specimens for laboratory analysis, takes and records TPR and weights.

* Measures and records intake and empties urinary drainage bags.

* Performs simple treatments.

* Reports suspected abuse/neglect to Team Leader, Manager of Nurse Services and/or Nursing Care Coordinator.

* Assists with admissions, transfers and discharge of Residents.

* Assists with orienting new residents and family members to floor routine.

* Gives motivation and encouragement to residents.

* Participates in Team conferences and tour of duty reports.

* Prepares residents for appointments, medical tests, therapy & activities.

* Wash wheelchairs/clean resident shoes, as assigned by night shift.

* Maintains confidentiality of all Resident care information.

* Follows infection control procedures and isolation techniques as directed.

* Assists with post-mortem care.

* Conducts lounge activities & supervision as assigned.

* Responsible for the condition of all resident belongings.

* Keeps work areas clear & safe.

* Responsible for assigned fire duties and various emergency procedures.

* Participates in staff development programs on and off unit.

* Responsible to attend required annual in-service programs; (6) hours of in-house in-service every (6) months.

* Performs other duties as assigned by professional staff.

Qualifications and Background

The ideal candidate must have a High School diploma or General Education Degree (GED) with six month related experience, preferably after a 100 hour CNA program. Must have current/valid NYS Certification.
