This person works under the general direction of the VP Chief Nursing Officer. Incumbents are responsible for various shifts and may be subject to hours over 40 per week and call back as required.
- Assesses the age appropriate physical, psychosocial, educational, and environmental needs of patients/family in a systematic manner. Identifies high risk patients.
- Develops and oversees the services provided to the client throughout the continuum of care in order to decrease fragmentation. Acts as a liaison between the physicians and the healthcare team to ensure quality and continuity of care delivery.
- Assesses patient/family learning needs; develops, implements and evaluates education provided to ensure that learning needs are met.
- Promotes an interactive relationship that supports effective communication among patient, family and nurse. Ensures the patient’s right to choice.
