Develops and coordinates services provided patients dealing with colon and lung cancer throughout the continuum of care in order to decrease fragmentation.
Acts as a liaison between the physicians, the professional health care staff and other healthcare team members to ensure quality and continuity of care delivery.
This person works under the general direction of the VP Chief Nursing Officer. Incumbents are responsible for various shifts and may be subject to hours over 40 per week and call back as required.
Assesses the age appropriate physical, psychosocial, educational, and environmental needs of patients/family in a systematic manner. Identifies high risk patients.
