Homecare/Hospice Coordinator is responsible for coordination of homecare/hospice services between hospital and home care program. This position is involved in the delivery of home care to support the goal of continuous patient care services at a level consistent with the standards of the hospital and of the Agency's mission statement. Coordinates the care of patients who are candidates for homecare/hospice services, both adults and pediatrics. Interviews patients and their families to make nursing assessments, explain home care, and include them in the planning process. Reviews patient admissions with appropriate hospital staff for identification of potential home care candidates. Acts as a resource regarding home care to hospital staff, physicians, and patients, including a broad knowledge of reimbursement regulations. Attends and actively participates in discharge planning rounds in patient care units. Implements discharge plan through self and other members of the health care team, and makes appropriate agency contacts to ensure effective outpatient or home care needs are met. Provides pre-discharge education to patients as stated in hospital contracts. Communicates with patient billing office, utilization review coordinators, admissions and clinics regarding patient admissions and criteria and follow-up documentation of post-hospitalization needs. Verifies and processes referrals to the Agency, consulting with physicians to obtain changes and/or clarifications to orders as needed. Acts as an on-going liaison between field staff, hospital, and clinic. Follows up with patients and referral sources regarding customer service. Educates new employees, physicians, students, and other interested parties to the philosophy of home health care. Promotes home care advantages to the community. Actively & appropriately markets the Agency to referral sources and potential clients with-in the boundaries of State & Federal regulations. Represents the Agency through quality work, accuracy, reliability, and clear communication.
Completion of an educational program for Nursing. Must have an active Colorado RN license or a license from the state of primary residence, if participating in the NLC (Nurse Licensure Compact). Twelve months nursing experience required; six months home care experience or case manager experience strongly preferred. Knowledge of principles of home health nursing, discharge planning, and continuity of care to provide in-services and hospital education, or to serve on Agency/hospital committees as requested. Knowledge of, or willingness/aptitude to develop, strong customer service and marketing skills. Strong leadership skills and creativity. Can read and interpret physicians' orders and notes as well as respond to them, verbally and in writing. Able to motivate physicians and hospital staff to focus on pre-discharge patient planning.
