The HOPWA/ESG Coordinator will provide housing assessment, care planning referrals, re-evaluation, and discharge planning to individuals participating in state grant-funded programs, such as HOPWA or ESG/RR Programs. This may include evenings, weekends, and other non-traditional hours. Desirable Qualifications Bachelor's degree in behavioral science highly preferred; equivalent experience considered One (1) year of case management experience required Knowledge of and ability to relate to community resources Good communicator with strong written and oral skills Good presentation skills Ability to maintain strict confidentiality Ability to work and effectively deal with multiple types of personalities in a variety of situations Professional presentation Must be self-motivated and research oriented Must possess and demonstrate good problem solving skills The nature of work may generate considerable physical (transporting, moving, lifting, etc.), mental and visual demands. Must be able to lift 25 lbs overhead and a maximum of 50 lbs and must also possess/maintain a valid driver's license, current automobile insurance and a driving record that meets the guidelines of the organization. Annualized starting salary does not exceed $25,000 and is based upon relevant experience.
