The Care Coordinator is responsible for facilitating the patient’s hospitalization from preadmission through discharge. The Care Coordinator coordinates with physicians, nurses, social workers, and other health team members to expedite medically appropriate, cost effective care. The Care Coordinator applies clinical expertise and medical appropriateness criteria to resource utilization and discharge planning. S/He will advise the healthcare team and provide leadership as needed.
Bachelor’s Degree in Nursing or a related field.
Current RN license in the state of Connecticut.
Must have a minimum of 4 years experience in utilization review or case management.
Strong written and verbal communication skills.
Strong interpersonal, managerial and educational skills.
Knowledge of clinical and fiscal interactions across the continuum of care.
Strong working knowledge of Medicare and reimbursement plans.
In-depth skill and medical knowledge to manage patient care and maximize timing of sequence of activities.
Ability to conduct comprehensive patient assessment and analyze data to formulate an appropriate plan for patient management.
Case management certification or eligibility for certification is preferred.
Master's Degree preferred.
16 hours every Saturday and Sunday; 8:00am to 4:30pm.
