The RN Case Manager is a member of the Home Hospice
Interdisciplinary Team (IDT), which identifies and provides the
appropriate patient/family needs during end-of-life care. These RN
Case Managers are responsible for managing sophisticated, high-
tech and high compassion therapies which include the most recent
palliative care standards. Care of these patients will involve
performing skilled nursing assessments, providing pain and
symptom management and applying appropriate interventions
in efforts to maximize the comfort and health of patients and
families throughout the patient illness continuum.
BSN and current PA RN licensure required. Candidate must possess at least one year of clinical hospital based or hospice/home care experience. Incumbent must have a current, valid driver's license and car insurance, and be able to drive long distances.
This person must be able to function effectively under stressful situations with tight deadlines; be a self-directed, self-starter; demonstrate creative thinking ability, as well as the knowledge and skills necessary to provide the expected standards of care; demonstrate the professional skill and judgment necessary to provide care within the autonomous setting of the patient’s home or at the various sites of hospice care; attend to the values and rituals important to the patient-family-community environments as well as upholding nursing practice and safety standards.
