CASE MANAGER/CLIENT COORDINATOR

RESPONSIBLITIES

Plan, implement and supervise creative curriculum for individual and group activities, community integration experiences and Residential L.I.F.E. Services Programs to facilitate functional participation as indicated in individual service treatment plans. Provide case management to LIFE Program and act as liaison to all external resources and provide necessary documentation .

REQUIRED QUALIFICATIONS:

  • A Bachelor's degree in a human services, recreation or related field is required.
  • A personal vehicle with valid Wisconsin Driver’s License and proper liability insurance in accordance with agency policy is required.
  • Documented experience working with people with developmental disabilities, mental illness and/or older adults or related applicable experiences transferable to program needs.
  • Experience working with people with challenging behavior.
  • Experience developing and implementing curriculum preferred.
