Reporting to the Executive Director, posseses in depth knowledge, competencies and skills in clinical specialty. Develops, teaches, coordinates and evaluates educational programs in collaboration with agency leadership. Functions as a leader and change agent through direct and indirect patient care, mentoring, consultation, clinical nursing research, program planning and education of patients, families and staff. Analyzes homecare/hospice practices to improve patient care. Establishes goals and priorities in accordance with patient and staff needs and organizational objectives.
5 years clinical experience in Homecare/Hospice
Masters degree in nursing with a concentration in related clinical specialty or equivalent experience
RN license - NH or other Compact State
BCLS within 30 days of hire
Driver license - valid
Serves as role model of professional nursing practice in homecare. Facilitates an empowered environment that recognizes and responds to psychosocial, development and professional practice issues through mentoring, group support, personal consultation, team building, resource identification and networking.
Plans, directs, organizes, implements and evaluates the educational requirements/needs for physicians and other healthcare providers as related to homecare/hospice. Assists Staff Development Specialist in the education of staff related to best practice initiatives.
Identifies areas for clinical studies and promotes or conducts investigations in these areas; incorporates current research findings into policies, procedures and standards.
Assists Staff Development Specialist in the education of staff related to best practice initiatives. Mentor and precept staff. Develop and coordinate competency training. Conducts joint visits with staff for competency assessment. Coordinates educational programs and outside speakers.
Leads in the development, implementation and education of Standards of Care policies and procedures for the agency. Assist with the development of patient education materials.
Analyze agency clinical outcomes, train staff, track progress and report to agency and Sr. Management at IOP. Collaborate in clinical agency QA activities and performance improvement to ensure high quality outcomes. Participate in meetings to develop appropriate clinical education based on quality outcomes.
Actively participates on specified clinically focused committees/task forces and acts as liaison for EHR continuum of care initiatives.
