At Pali Momi

residents of the West Oahu community quality medical care delivered

catheterization
program in West Oahu and the state’s only Retina Center. The

medical care and excellent service. The only diagnostic and
interventional cardiac catheterization program in West O‘ahu.

coronary

Your role as
Cardiac Services Referral Coordinator will be to serve as the primary
point of
contact for referring physicians, cardiac providers, and patients
throughout
the transfer process.

Provide essential
coordination of clinical activities for patients originating from a
variety of
settings.

Duties will also include
performing data entry, developing data quality reports, assisting
with patient
service excellence program components, and post procedural phone
calls and
follow-up.

Requirements

Minimum

Qualifications:
High school diploma or equivalent.

One year experience working in an outpatient
clinic setting.

Preferred:
Associate’s degree.
