IMPACT Plus Clinical Case Manager
KVC is seeking clinically licensed Master’s level clinicians who desire to make a difference in their community. We are in need of a full-time Case Manager and Behavioral Health Specialist or Behavioral Health Specialist under supervision manage a case load and provide treatment to our clients. KVC professionals work in-home with families and children who are in need of intensive in-home, behavioral treatment or therapy services.

With the ability to work flexible schedules and in the homes, schools and communities of the families we serve, this position lends itself well to the on-the-go, self-starter who has a passion for making a difference. Because each member of our team is supported by a local area Director and support team, IMPACT Plus professionals will not only have clinical supervision available for licensure, but will also have access to a breadth of knowledge of the community resources available to our staff and our clients.

This position requires the skill sets of both a Case Manager and a Behavioral Health Specialist or Behavioral Health Specialist under Supervision. The employee will assume a combined caseload of clinical and case management cases equaling one full-time position. This position requires a Master’s degree in Social Work, Counseling, or Psychology. Certification or licensure through the State of Kentucky as master's level professional designated field is required.

Applicants must be certified or licensed in clinical social work, professional counseling or marriage and family therapy; or able to be provisionally licensed in the same fields to provide clinical services in the state of KY. All candidates must maintain professional licensing and certification as identified by the supervisor or agency director and state regulations.

Job duties may include the following as well as other duties in order to meet the needs of our clients and families.

IMPACT Plus Clinician:
a. Conduct psychosocial assessment and other paperwork as dictated by Chart Online and 907 Kentucky Administrative Regulation 3:030.
b. Collaborate with Service Team to author Plan of Care.
c. Develop and maintain a comprehensive treatment plan.
d. Provide individual and/or collateral therapy as dictated by the Care Plan.
e. Attend Service Team meetings and collaborate in treatment delivery.
f. Assist Case Managers and TCS in treatment delivery.

IMPACT Plus Case Management/Service Coordination:
a. Case Management/Service Coordination (as dictated by 907 Kentucky Administrative Regulation 3:030)
b. Coordinates service assessment.
c. Assists in developing, coordinating, and accessing services in the collaborative service plan (Care Plan).
d. Coordinates collaborative team meetings to develop, modify, and review a collaborative service plan (Care Plan).
e. Maintain communication with community partners, DCBS, Schools, Hospitals, and all other referral sources from Intake to Discharge.
f. Facilitates the implementation of a care plan.
g. Documents a minimum of four (4) contacts per month per client on separate days, including one (1) face-to-face 30 minute contact with a client and one (1) face-to-face 30 minute contact with a parent or guardian or primary caregiver.
h. Note: Once every two months, a telephone contact may substitute for a required face-to-face contact with a client if a client is located greater than 90 miles from the employment location of the client’s selected service coordinator.
i. Monitors the client’s progress and compliance with treatment.
j. Advocates that the client receives appropriate, timely, and effective support
k. services.
l. Participates in the development of other human service plans for a client.
m. Develops a plan of transition from IMPACT Plus services to a lower level of care for a client.
n. Provides the client with a list of all IMPACT Plus enrolled subcontractors
o. authorized to provide a service, pursuant to collaborative service plan, for the
p. purpose of selecting a provider.
q. Provides the client with information about availability of a service if an IMPACT Plus service is not available.
