The Admissions RN Coordinator, under the supervision of the Admissions and Palliative Care Team Leader, is responsible for supporting agency census by managing the day to day processes of Agrace admissions and referrals. RN will monitor progress of all open referrals and work with the Admissions Coordinators to convert referrals to admissions as soon as appropriate. Tasks may include:
  • Utilizing access to Electronic Health Records to obtain documentation related to hospice eligibility
  • Collaborate with Hospice and Palliative Physicians regarding eligibility for services
  • Provide back up to the Admissions & Palliative Care Team Leader during absences
  • Consult with Admission Coordinators to maximize scheduling of Admissions and Palliative Care teams
  • Delegate tasks/assignments as needed to meet the needs of the department/patients and families
  • Coordinate after hours/excess admission visits in order to accommodate pt and family needs
  • Complete admissions across settings as needed
  • Obtain clinical report from clinical referral sources
  • Provide clinical support to non-clinical Admissions staff
  • Assist staff unfamiliar with admissions processes
  • Orient new team members to Agrace admissions
  • Obtain insurance authorization for services prior to admission
  • Complete admissions to Agrace services
  • Attend committees and workgroups as assigned
  • Collaborate with Admissions team regarding challenging referrals
  • Provide follow up phone calls to palliative care patients and referring providers
  • Provide follow up to customer and partner concerns

The Admissions RN Coordinator works Monday-Friday 8-5 and as needed to meet patient and family needs. Extended hours and weekend rotation may be required. High level of flexibility, reliability, and autonomy are essential.
