Company Description VNS CHOICE Community Care is a managed long term care program for frail older adults who wish to remain in their own homes to receive long term care services for as long as possible, but require ongoing assistance due to chronic illnesses and functional decline. The members of the program's highly collaborative care management teams include registered nurses and nurse practitioners, rehabilitation therapists, licensed social workers, and nutritionists. These professionals coordinate and/or provide home-, community- and facility-based long-term care and health-related services. We currently service more than 3,600 members in all five boroughs of New York City.We are seeking talent for the opportunity described below.

Responsibilities Responsible for managing interdisciplinary health care teams to ensure that standards for services delivery and team/staff performance are met or exceeded. Provides administrative and staff support to the regional operations of VNS CHOICE. This includes supporting regional teams on a day to day basis related to care management issues, problem resolution, complaint resolution and other service related issues. Manages and coordinates the activities of the VNS CHOICE team by ensuring that the appropriate high quality of patient care provided is consistent with Agency mission, principles, policies and standards.

Works under general supervision. Qualifications Licensure: License and current registration to practice as a Registered Professional Nurse, Licensed Social Worker or Physical Therapist in New York State required.Education: Bachelors Degree or equivalent experience in Nursing or Health Care Administration or Masters Degree in Social Work or Masters Degree in Physical Therapy required. Masters Degree in Nursing or Health Care Administration preferred. Case Management Certification preferred.Experience: Minimum of three years experience in home care, care management or related field required.

Minimum of three years of progressive experience in Clinical Management required. Effective oral/written communication and interpersonal skills preferred.
