Opening Available Title: Health Director Job Summary: Plans, directs, and coordinates participant care activities in Medical Adult Day Services to maintain standards of participant care, and advises medical staff, department heads, and administrators in matters related to nursing service. Complies with COMAR regulations. Supervises activity of Nursing staff.

Job Duties and Responsibilities: Recommends establishment or revision of policies, procedures, processes and standards of performance. Advises and assists Executive Director, Medical Director, and Nursing staff in planning and developing administrative and operational policies, procedures, processes, and strategic planning. Coordinates Nursing activities to include: Confers with managers, staff, and family/caregivers concerning problems of participant care Evaluates the education and training needs of personnel Promotes cooperative working relationships with hospitals, community agencies, and with other departments Initiates studies to evaluate effectiveness of nursing services in relation to their objectives, costs, and outcomes Resolves complaints Completes admission paperwork in a timely fashion Participates in care plan reviews and initiates care plans changes Performs assessments and completes Adult Day Care Assessment and Planning System (ADCAPS) nursing sections on all participants every month Establishes Adult Day Care Assessment and Planning System (ADCAPS) evaluations on the date of admission and completes assessment with an initial of care within 30 days then on a quarterly basis to comply with COMAR regulations Completes quarterly Adult Day Care Assessment and Planning System (ADCAPS) assessment Has a quality assurance program which includes health care audits and utilization reviews to comply with COMAR regulations Dispenses medications according to physicians orders Initiates and completes interdisciplinary care plans and paperwork on new and current participants to include type and duration of service, frequency of service, dosage, frequency of medications, and other information as set forth by COMAR regulations Charts findings in progress notes and maintains record of medical history, chronic illnesses, principal and significant diagnoses, prognoses, prescribed medications, special diets, allergies, and assessments of recipients physical and mental status specifying the general types of activities the patient can and cannot do, another pertinent information as set forth by COMAR regulations Has an emergency plan for each participant and ensures that such is easily located on each participant's file including advance directive and other information as set forth by COMAR regulations Schedules clinical and provider appointments as needed Arranges to give flu shots every Fall - Oct./Nov. for participants Performs other duties as assigned Qualifications Experience: 5-10 years in Adult Day Care Setting, Case Management, and/or Home Health Care.

Education : Registered Nurse in State of MD. Active RN license. Competencies: Leadership, organizational and time management skills. Ability to establish rapport and to assist client to identify his or her own needs (i.e., self-care skills, social skills, independent living skills, medication management, health- mental and somatic, entitlements, and vocational) to establish appropriate rehabilitation goals and to provide internal and/or community referrals when necessary.

Ability to speak more than one language preferred. Good written and oral communication. Technical Skills : Maintains current and active CPR/First Aid, Infection Control, Food Safety and Handling, and Crisis Intervention/Suicidal Protocol. A Current, valid driver's license, proof of auto insurance (which meets minimum MD state requirements), and a current driving record are also required for transporting consumers, either in agency vans or a personal vehicle.
