GENERAL SUMMARY:
Under the direction of the EMR Abstraction Lead, reviews paper medical record for pertinent information (i.e. patient allergies, medications, past medical history, family/social history and procedures) that will be abstracted for the electronic medical record. Enters information found into the electronic medical record (EMR) software.

ESSENTIAL JOB DUTIES:
Reviews and analyzes paper medical records to identify pre-defined, pertinent patient information for abstraction into the EMR.

Performs accurate and complete abstraction of patient medical records following EMR abstraction guidelines.

Consults with provider staff and/or EMR Abstraction Lead if questions arise regarding information to be abstracted.

Performs accurate scanning of identified documents into appropriate section of the EMR as needed

KNOWLEDGE, SKILLS AND ABILITIES:
Knowledge of general office practices and procedures.

Knowledge of proper grammar, spelling, and punctuation.

Knowledge of Microsoft Office Suite Software.

Knowledge of medical terminology.

Skill in operating a computer and various software packages.

Skill in operating office equipment such as copier, fax machine, telephone, etc.

Ability to analyze medical record information to determine information to be abstracted.

Ability to speak clearly and concisely.

Ability to speak, read, understand and follow oral and written instruction in English.

Ability to establish and maintain effective working relationships with physicians, patients, co-workers and the public.

Ability to perform computer tasks at a user level including efficient typing skills.

Strong attention to detail.

Principal Duties and Responsibilites:
1. Maintains on-site storage unit which contains patient charts in boxes and/or on filing shelves including alphabetizing and filing reports into patient charts; and pulling and delivering charts as requested by all departments.

2. Uses computer inventory systems to track and maintain charts. Chart tracks record to appropriate location.

3. Coordinates with off-site storage company to deliver charts between the office and storage facility.

4. Maintains current patient charts including creating, splitting and purging as needed.

5. Responds to overhead pages and all medical records phone calls specific to chart needs.

6. Takes requests from other departments for delivery of boxes/items that are in the on-site storage unit.

7. Maintains a highly ordered system with low tolerance for errors.

Qualifications

EDUCATION AND EXPERIENCE:
High school graduate or equivalent plus a graduate of an accredited medical program such as Practical Nursing, Registered Nursing or other clinical health field is required. Current licensure and/or certification associated with degree also required.

Work requires:
Prolonged sitting.

Occasional walking, bending, and stretching for files and supplies.

Requires manual dexterity sufficient to operate a keyboard, telephone, fax machine, copier and other office equipment as necessary.

Vision must be correctable to 20/20 and hearing must be within normal range for telephone contacts.

It is necessary to view computer screens and type for long periods of time.

Light lifting of up to 10 pounds frequently and up to 20 pounds occasionally.

Normal office environment.
