UPMC Community Provider Svcs is hiring a full-time Supervisor Nurse Liaison to help support the HOS00 LIAISONS Department for its Oakland and various locations location.

The RN Liaison Supervisor acts as the key facilitator to assist the liaison staff and hospital in the discharge planning process. Monitors processes for the transition of the patient, from the hospital to the home setting, to achieve positive patient outcomes. Acts as an educational and consultative resource to the liaison staff and institution concerning appropriate home care services and community resources.

Responsibilities

- Maintain the standards of excellence and act as an educational and consultative resource to the
liaison staff and institution
- Directly supervise assigned liaison staff
- Provide input to Director regarding hiring and termination of staff
- Complete and present staff performance evaluations
- Execute disciplinary actions
- Directly interact with other departments to resolve issues and ensure that policies are being
followed
- Educate the liaison and institution staff regarding the In Home Services Network and services
available to assist the patient in the home setting
- Educate and serve as a resource to the liaison staff and institution staff regarding standards of
care and regulatory requirements as they impact home care services
- Address and resolve issues and concerns with physicians and institution staff as they relate to
the patient's discharge plan
- Maintain open and positive communication with all contacts
- Participate in the institution and UPMC Health System educational activities as a community
services resource and educator
- Participate in the institutions community programs as requested and needed by the institution
- Maintain service excellence with internal and external contacts
- Screen on-site operational issues and report them to director
- Participate in process improvement committees with home care staff and institution staff
- Maintain time and attendance records for direct report staff
- Maintain staff records to monitor licensure, mandatory training and in-service attendance
- Monitor staff productivity
- Act as back up contact for on call staff
- Adhere to company and departmental standards, policies and procedures
- Follow established administrative and human resources policies and procedures; to ensure
compliance at all times
- Comply with accepted ethical conduct and professional Standards of Nursing Practice
- Promote and practice the UPMC Health System's vision statement
- Promote and practice the UPMC In Home Services Network mission, vision and values
- Maintain patient information and record confidentially as outlined in departmental and institutional
policies and procedures
- Adhere to departmental and institutional policies and procedures in maintaining a safe work
environment
- Develop, implement, and evaluate new processes relevant to providing quality referrals and
monitor compliance with same
- Attend educational in-services, departmental staff meetings, and mandatory training and UPMC
Health System meetings as required
- Assist in the training and orientation of new Outpatient Registered Nurse Liaison staff
- Act as conduit between liaison staff and liaison director
- Assist as preceptor for on-site orientation of new staff
- Provide educational in-services to on-site liaison staff
- Screen on-site operational issues, and report them to director
- Act as conduit for process improvement suggestions and concerns

Basic Qualifications

- Completion of an accredited School of Professional Nursing required
- BSN preferred
- 2 years of hospital experience required
- 1-2 years-home health experience and knowledge of home care qualifying guidelines required
- 2-3 years Nurse Liaison experience required
- 1-2 years Lead Liaison experience required
- Excellent written, verbal communication, and interpersonal skills required
- Adaptability and flexibility to change
- Ability to handle multiple tasks in a fast paced environment and to function independently
- Ability to analyze issues objectively, provide workable solutions and implement these actions
