Care
Transitions Project Manager

The Care Transitions Project Manager will apply
the latest quality improvement tools and techniques to assist
community based teams with improving processes for transitioning
patients between settings of care.

One goal of the Care Transition project is to
reduce hospital readmissions. The PM will provide consultative
services to providers within community teams related to analyzing
readmissions data, performing root cause analysis, selection and
implementation of evidenced based interventions, and collecting
process measure data to monitor effectiveness.

The PM will also manage state-based Learning
and Action Networks to facilitate large scale improvement.

Requirements

Bachelor's degree and five years related
experience and/or training or equivalent combination of education and
experience required, Master's degree and three years of direct
project management experience preferred. Desired course of studies
may include healthcare or related field

Clinical background
preferred

Must be able to travel between healthcare
providers including hospitals, nursing homes, physician office or
other healthcare settings in Maryland and DC

Applicants must
demonstrate the ability to work independently, manage multiple
priorities, and design and implement improvement plans of action

Subject matter
expertise in healthcare, quality improvement and project management
methodologies required

Successful
candidate will have advanced presentation skills, including
experience with speaking in front of groups, large and
small

Community
organization or coalition-building experience is a
plus
