Be the key liaison between the patient and their Patient-Centered Medical Home team, including providers, clinic staff, and other health care professionals.
  • Ensure safety, best practices, and high quality standards are maintained for the patient across the health care continuum.
  • Resolve urgent or time-consuming tasks that improve quality, team efficiency, and patient or office flow.
              • Required qualifications for this position include:
                • High school diploma or GED. Some college preferred.
                • Current certification from the AHA (American Heart Association) for BLS (Basic Life Support) for the Healthcare Provider.
                • If position is in Washington State, applicant must meet requirements for certification according to RCW 18.135, State of Washington, and Department of Health.
                • Minimum of five years recent experience typically found working as a Certified Medical Assistant or LPN (Licensed Practical Nurse).
                • Previous experience leading others strongly desired.
                • Computer skills including Microsoft Windows and Office Applications.
                • Experience with electronic medical records and/or computerized billing systems.
