Be the key liaison between the patient and their Patient-Centered Medical Home team, including providers, clinic staff, and other health care professionals.
Ensure safety, best practices, and high quality standards are maintained for the patient across the health care continuum.
Resolve urgent or time-consuming tasks that improve quality, team efficiency, and patient or office flow.

About Providence in Oregon
- High school diploma or GED. Some college preferred.
- Current certification from the AHA (American Heart Association) for BLS (Basic Life Support) for the Healthcare Provider.
- If position is in Washington State, applicant must meet requirements for certification according to RCW 18.135, State of Washington, and Department of Health.
- Minimum of five years recent experience typically found working as a Certified Medical Assistant or LPN (Licensed Practical Nurse).
- Previous experience leading others strongly desired.
- Computer skills including Microsoft Windows and Office Applications.
- Experience with electronic medical records and/or computerized billing systems.
