Patient Placement Center Task Overview

Coordinate and track all referral hospital transfers

' Assist in smooth transfer into RB or UD by outside facilities

' Assist in getting patients directly admitted to Cath Lab or Surgery when physician requests

Placement of all inpatients at SHMC RB & UD

' Direct admissions

' ED Admissions

' Transfers-within hospital, between districts, and from other facilities

' OR/PACU-Post-op patients requiring inpatient placement using SmarTrack

' OHVI Cath Lab-Post-procedural patients requiring inpatient placement

' OCDC-Post-procedural patients requiring inpatient placement

Coordinate and track transfers between UD & RB districts including arranging transportation

' Specialty Units

' SPR Appointments

' Procedures

' Admissions

Work collaboratively with Radiology Departments, internal and external, to integrate outside images to internal EMR

Physician Services: MD's are encouraged to utilize the PPC for all requests internal or external

' Manage ED Call Schedule for both RB & UD-responsible for entering and editing call duty for all panel participants/groups including changes to posted schedules

' Submit monthly summary of call hours for billing purposes

' Maintain 'undoctored' rotation lists for Family Practice and Internal Medicine

' Facilitate physician communication and consultations

' Provide direct or back-line contact numbers for physicians

' Assist in paging and facilitating call back for physicians

' Assist physicians and their staff with patient location

Manage all hospital expirations & Outside ME Cases

' Track location and movement of all bodies (morgue, pathology, tissue bank, funeral home, etc.)

' Maintain all paperwork associated with expirations (Release of Body, Autopsy Consent, Organ Donation)

' Monthly reporting to State of Oregon

' Final release of bodies to Funeral Homes

HAN/HOSCAP Administration for UD and RB

' Liaison with State of Oregon regarding program, web site, internal user training/account maintenance, etc.

' Responsible for updating HOSCAP every shift, both facilities, including total number of available beds and ventilators

Monthly Reporting of Consults and Admissions from Outside Facilities

Facilitate communication with 'Nighthawk Radiologists' and the OC Hospitalist

Enter Surgical add-on cases into Smartrack after normal business hours

Care Manager Support

' Daily reporting to Care Managers notifying of outside transfer patients

' Provides physician name, in rotational listing, from the IM Panel for 'Undoctored Patients'

Place Series Patients from Willamette Valley Cancer Center

Monthly reporting to ED Leadership Team on interfacility transfer tracking

Approve and pay ambulance invoices for applicable transfers between facilities

Daily communication to Interpreter Services regarding inpatients with special need requirements

Coordinating Palliative Care consults with physicians in the community and the Hospitalists

Working with Medical Records to facilitate patient charts being scanned in a timely manner for patients transferring to BHS or Rehab from RB

Collaborate with Admitting to assure patient is admitted with correct account status, and communicate patient placement through the Direct Admit Board to they can receive and direct patient upon arrival

Night Shift Patient Placement Coordinator will also perform the essential functions of a Staffing Coordinator being responsible to adequately staff all inpatient units at Riverbend and University District.

Qualifications:
Current Patient Placement or Staffing Coordinator experience preferred.
