Develops program model for direct implementation of the program objectives. Teaches and implements individual program goals based on individuals' needs. Maintain yearly plans (addenda) which outlines the specific objectives and teaching methodologies to be used with each client throughout the year. Participation as member of Interdisciplinary Team; records random data of individuals; computes data monthly and forward to the Service Coordinators by the 10th of each month. Attends all team reviews for clients serviced. Administer a yearly diagnostic assessment on each client serviced to make the recommendations to the Service Coordinator based on results. Compiles monthly attendance for Medicaid purposes; computes accurate wage/payroll records on assigned caseload. Plan daily lesson plan and prepare materials; attends weekly staff meetings; attends workshops designed to develop skills. Maintain equipment inventory.
Minimum and Additional Requirements:
Bachelor degree in Human Services field with three years experience in providing services to people with disabilities or the equivalent in education and experience.
