Job Responsibilities RN NICU Case Manager will work with NICU members and families to assess their care needs while they are inpatient at the hospital. RN NICU Case Manager will assist in planning and implementing interventions, assist in coordinating services for discharge, and provide continued telephonic outreach after discharge in order to assess patient needs, offer support, and educate. You will collaborate with other health care providers in reviewing actual and proposed medical care and services. As an RN NICU Case Manager, you will examine clinical programs information to manage our members on an outpatient basis and develop strategies to prevent readmission; to reduce non-emergent Emergency Room utilization; and conduct discharge planning. You will conduct post-discharge calls and discharge planning for up to one year after discharge. This position may require frequent onsite visits to hospitals and/or other healthcare facilities, and the ability to work via cable or DSL modem from home in a secure environment where confidentiality of member and company information can be maintained. Will work independently under general instructions and with a team.
3-5 years of clinical experience as an RN in a Level III NICU. Bachelors degree in any field. Current RN license in the state of Wisconsin with the ability to obtain RN license in multiple states. Excellent written and verbal communication skills. Valid drivers license and/or dependable transportation necessary. Ability to work/train in Louisville, KY for 1-2 weeks, plus additional travel as needed for ongoing training.
