The Social Worker provide clinical psychosocial and case management services at an advanced practice level to eligible veterans and their family members to meet biological, psychological, social, and environmental needs.

Incumbent will coordinate services with the community's homeless providers and coalitions as well as with other local, state, and Federal agencies. The incumbent will be responsible for the Veterans Housing and Urban Development-VA Support Housing (HUD-VASH) program coordination and development, plus primary responsibility for the provision of or coordination of the VA's intensive case management services to those veterans utilizing the HUD/VASH housing vouchers. The primary goal of HUD/VASH is to move veterans and their families out of chronic homelessness. Incumbent will be responsible for the assessment, identification, facilitation and referral to appropriate medical and psychiatric services to those veterans identified as appropriate for the HUD/VASH program. He/she will evaluate the veteran's situation, abilities, and capabilities, and arrive at a reasoned conclusion including an assessment of vulnerability and prioritization for admission. Incumbent will assess at-risk factors and develop a preliminary plan, involving the veteran and family or significant others, and perform an insightful assessment of serious complicated cases involving psychiatric illness which may also include, catastrophic medical condition, dementia, traumatic brain injuries, and other high risks diagnoses. As part of a comprehensive psychosocial assessment, the incumbent will interview the veterans situation, presenting problems and their causes, and the impact of such problems on the veterans functioning and health. The incumbent will review all data, subjective and objective and make a clinical assessment, identifying needs and strengths. Based on the psychosocial assessment, the incumbent will use professional judgment and advanced practice skills to make a psychosocial diagnosis. Incumbent will provide referral to VA medical and mental health inpatient and outpatient treatment and to other VA services including VA benefits, as well as community-based social services and other non-VA entitlement programs. The incumbent will establish procedures and coordinate the screening and admission process for the HUD/VASH program by developing and maintaining criteria that recognizes program eligibility, prioritization for program acceptance, and requirements of the public housing agencies. The incumbent will be responsible for clinical social work practice in the area to which assigned, including promotion of social work services as a key component of treatment. The incumbent will organize community services on behalf of homeless veterans developing and coordinating procedures for use of these services by related staff. He/she will provide direct mental health and substance abuse services for veterans and family members or significant others in support of the veteran's treatment. The incumbent will establish a continuing relationship with the veteran, evaluate progress towards goals and adjusting the treatment plan as appropriate. The incumbent will be responsible for data collection, administrative record keeping, and monitoring patient or program clinical indicators, thresholds, and performance measurements.

Specialized Experience. Must have experience that demonstrates possession of advanced practice skills and judgment. At least two years should be in an area of specialized social work practice.

Licensure: M ust be licensed or certified by a state at the advanced practice level (LCSW or equivalent) which included an ASWB advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice). Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty.

Education: The basic requirements for employment as a VA social worker are prescribed by statute, Title 38 United States Code (U.S.C.) §7409(b)(9), as amended by section 205 of Public Law 106-419, November 1, 2000 All education claimed by applicants will be verified by the appointing agency accordingly. Have a master's degree in social work (MSW) from a school of social work fully accredited by the Council on Social Work Education (CSWE).

Education must be accredited by an accrediting institution recognized by the U.S. Department of Education in order for it to be credited towards qualifications (particularly positions with a positive education requirement.) Applicants can verify accreditation at the following website: Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work.
