The Speech Language Pathologist is responsible for provision of speech therapy within the regulations of the American Speech – Language – Hearing Association and the NC Board of Speech and Hearing to include patient evaluation, administration/supervision of treatments, teaching, documentation of services in the patients medical record, appropriate communication/conduct.
Essential Duties and Responsibility Levels:
A. Provides SLP services to include evaluation, treatment, reassessment, discharge planning and patient and family education within regulation and according to physician orders.
B. Documents accurately, objectively and timely in accordance with regulations and agency policy /procedures. Documentation includes but is not limited to assessment, treatment plan, provision of care and services, discharge summary, certifications and billing.
C. Follows departmental and service policy and procedure.
D. Meets individual productivity standards.
