The ambulatory care RN works with defined populations seeking care for health promotion, health maintenance, or health related problems. The role of the RN is to skillfully assess the patient’s health and response to interventions, provide treatments, and offer education and counseling that lead to improvement of the patient/family’s self management skills. The RN evaluates the effectiveness of care in the context of the patient/family’s goals and lifestyle and takes necessary action to promote optimum outcomes. Nursing actions are based on principles of respect, advocacy, research and evidence-based practice, interdisciplinary collaboration, clinical coordination/care management, leadership, and outstanding communication and customer service. Nursing practice is relationship-based and can occur in single or ongoing encounters, in person, telephonically or using computer-based communication. Nursing serves patients ranging in age from newborn to elderly. Position requires high level of customer service skills to establish and enhance positive relationships with patients, co-workers, and others.

The Nurse Clinician/ MSHO Care Coordinator coordinates the provision of all Medicare and Medicaid health and long-term care services for MSHO enrollees, and coordinates services to an MSHO enrollee among different health and social service professional and across settings of care.

JOB QUALIFICATIONS/EDUCATION/EXPERIENCE REQUIREMENTS:

Education/Experience:

1. Graduate of an accredited professional nursing program, eligible for RN licensure.

Licensure/Credentials:

1. Current registration or permit to practice professional nursing in appropriate state(s), i.e. Minnesota, Wisconsin or Michigan.

2. BLS or ability to attain within 3 months.

3. Must have reliable transportation to work sites and valid driver’s license
