MENTOR Oregon is seeking a Case Manager to meet and work with individuals to gather information and assess each client/consumer’s needs to include the assessment of the cost of care. The Case Manager may prepare a plan of care, and identify and implement services to meet an individual’s needs.
Bachelors degree in Human Services or equivalent in education and experience required
In addition, one year of related work experience and knowledge of case management
Other education and experience plus licensure as required by state
