Monday - Friday Day Program
Full-Time 8am-4pm

The Team Leader is responsible for guiding the daily operation of the program. This responsibility includes training and supporting the team, overall program development and implementation, and management of program budget, materials and space. The Team Leader is responsible for working with the team to create and foster an environment of consumer and staff empowerment. The Team Leader also assumes overall responsibility for ensuring active daily treatment and full compliance with all DMA and DDS regulations as outlined. The Team leader should promote community inclusion and self advocacy.

Responsibilities

  • Maintain oversight of case management services as well as coordination of active daily treatment for all program consumers.
  • Ensure consumer treatment and service coordination service needs are met through the consumer's inter-disciplinary team as outlined in the Division of Medicaid regulations regarding active treatment. This includes maintaining daily staffing ratios to ensure compliance with Medicaid regulations and DDS supplemental staffing requirements, as well as providing direct support coverage when necessary.
  • Ensure continuous quality improvement of individualized programs and services through the use of both internal and community resources. Assist team members with the development of teaching curriculums designed to promote individual growth and increased independence. Teaching methods should include available community resources as a viable strategy to achieve individualized and program goals.
  • Ensure the development of appropriate consumer - centered objectives for all program consumers within his/her area of responsibility with emphasis on identified need areas, as well as community inclusion.
  • Coordinate scheduling and use of all consultant time with other team leaders. Coordinate and ensure carryover of therapeutic recommendations into daily schedules.
  • Ensure completion of initial and annual assessment of program consumers using community mobility, safety skills, fine motor, life skills, and ADL assessment tools. This includes completion of annual DHSPs, DMA Severity Profiles, daily notes and monthly summaries and all other required documentation as outlined in the Medicaid regulations. Ensure all consumers have an up-to-date case record.
  • Work with case managers, skills instructors and other team leaders to plan for and provide in-service training as is appropriate for consumer needs and team expertise.
  • Maintain open lines of communication with consumers, families, funding sources, consultants, the community at large, program and administrative team.
  • Ensure all consumer toileting and personal care needs are met.
  • Provide direct care as dictated by regulatory ratios (1:4-1:7).
  • Provide direct support coverage in absence of staff.
  • Ensure development and implementation of all Level I and Level II behavior plans as indicated through the DHSP process. Receive and maintain certification in the prevention and management of aggressive behavior.
  • Supervise and maintain performance evaluations for assigned team.
  • Ensure that individuals we support are treated with dignity and respect in accordance with Advocates' Human Rights Policy.
  • Attend trainings as assigned; maintain necessary certifications (CPR, First Aid, SOLVE, HR/DPPC).
  • Attend and actively participate in all team meetings and supervision.
  • Perform all duties in accordance with the agency's policies and procedures.

  • Qualifications

  • Bachelor's degree in a human services related field including psychology, education, special education, reaction therapy, adaptive physical education, rehabilitation counseling, social work or sociology and at least three years of demonstrated, related experience in case management and developing and implementing teaching strategies. Three years related experience may be substituted for a degree.
  • Must be able to perform each essential duty satisfactorily.
  • Basic computer knowledge.
  • Demonstrate strong oral and written skills (English proficiency) with consumers, families, funding sources, consultants, the community at large, program and administrative team.
  • Demonstrate an ability to work independently and as part of a larger resource team.
  • Must hold a valid drivers' license. Must have access to an operational and insured vehicle and be willing to use it to transport consumers.
