Primary Function & Major Responsibilities The one year Resident RN Cohort model allows new grad Resident RNs to acclimate to a HealthEast Home Care position through clinical orientation, professional development, clinical coaching and facilitated learning sessions.Provides nursing services utilizing comprehensive base of nursing theory and nursing process, and communicates/documents observations and assessments to appropriate.Delivers effective and appropriate home nursing care, establishing key relationships with patients and family members for the purpose of advocacy and coordination of care.Collaborates with RN Case Manager and other health professionals as appropriate in determining patient care goals.Educates the patient/caregiver how to self-manage their disease on a day-to-day basis in collaboration with the RN Case Manager.Recognizes and reports signs of impending physiologic and/or psychological patient changes to the appropriate physician and RN Case Manager to implement appropriate patient care interventions.Recognizes, evaluates and documents needs of patient and family to provide the most effective and appropriate patient home health care.Maintains confidentiality in all aspects of the job.Performs all other related duties as assigned.


BSN or BA degree in Nursing preferred; PHN Certification from MN Board of Nursing Preferred or PHN Certification MN Board of Nursing must be acquired within 6 months of the date of hire.

Must have less than one year of RN experience by cohort start date.Credentials: Life Support: BLS Certification AHANursing: Public Health Nurse MN Board of Nursing or Eligible to acquired within 6 months of the date of hire. Registered Nurse (RN) MN Board of NursingSpecial Knowledge, Skills, and Abilities: Collects and analyses data pertinent to patients health or situation;Implements and coordinates care following identified plan;Evaluates progress towards attainment of goals or outcomes;Provides patient educational information appropriate to patients developmental level, learning needs and readiness, cultural values and beliefs;Identify expected outcomes and develops individualized treatment plan of care;Integrates practices that support safety, acknowledges variations in the population served, and prevents harm to patients, families, other employees or themselves;Administers medications following policies using procedures that assure the highest level of safety all times for all populations;Demonstrates behavior consistent with the standards, qualities and characteristics of professionally licensed RN’s. This includes but is not limited to the current ANA Scope and Standards of Practice, Social Policy Statement and Code of Ethics for Nurses;Utilizes nursing process in providing holistic, compassionate, safe, high quality and population based care.Regular/Temporary
