Role: Clinical Innovations / Clinical Advisor, RN Assignment: NICU Register Nurse Case Manager Location: Milwaukee, WI Work-At-Home Are you a fit? You will get to know their families by providing case management, outreach, education and support in the most important year of their life. Assignment Capsule In this role you will work with NICU members and families to assess their care needs while they are inpatient at the hospital, assist in planning and implementing interventions to meet those needs, assist in coordinating services for discharge, and provide continued telephonic outreach post discharge to assess their needs and offer support and education. You will collaborate with other health care providers in reviewing actual and proposed medical care and services. You will examine clinical programs information to: manage our members on an outpatient basis and develop strategies to prevent readmission; to reduce non-emergent Emergency Room utilization; and conduct discharge planning. You will conduct post-discharge calls and discharge planning for up to one year post discharge. This work may require frequent onsite visits to hospitals and/or other healthcare facilities and the ability to work via cable or DSL modem from home in a secure environment where confidentiality of member and company information can be maintained. Role Essentials 3-5 years of clinical experience in NICU (Neonatal Intensive Care Unit) Level III setting Bachelor's Degree Ability to be licensed in multiple states with an unrestricted nursing license Active RN license in the following state: Wisconsin Knowledge of NICU terminology, care concepts and unique characteristics of a premature infant Excellent written and verbal communications Skilled in web navigation Must possess interpretive, intuitive and investigative skills Ability to work independently under general instructions and with a team Valid driver's license and/or dependable transportation necessary (variable by region) Ability to work / train in Louisville for 1 - 2 weeks, plus additional travel as needed for ongoing training Designated/ Secured work area to conduct business (I.e. Home Office) Ability to work flexible hours: Monday through Friday with a minimum of one evening per week Role Desirables Bilingual in Spanish language skills for both spoken and written are a plus Outstanding organizational skills
