A. LPN or RN is preferred.

B. Requires two to three years of related experience, preferably one to two years in home health, in order to understand appropriate resolutions for staffing and patient relevant problems.

C. Must possess interpersonal skills needed to interact effectively with patients, nurses, physicians, pharmacists, and in-house staff members.

D. Proficient typing and office skills necessary to assure accurate data entry, the preparation of reports and correspondence, and to maintain personnel files and staffing records.

Continuing Education Requirements: Organization personnel are expected to participate in appropriate continuing education as may be requested and/or required by their immediate supervisor. In addition, organization personnel are expected to accept personal responsibility for other educational activities to enhance job related skills and abilities. All personnel must attend mandatory educational programs

Job Description Summary:

Under the general supervision of the Clinical Manager/Clinical Director, assists and supports the clinical management team with the coordination of clinical services including field staff coordination, communication, and scheduling duties, chart and documentation reviews and tracking, admissions process, and community resource and customer service communication. Addresses, resolves, or communicates to appropriate supervisors, patient and staff relevant problems, using sound judgment and knowledge of agency policies, procedures, and regulations.

Essential Job Functions/Responsibilities:

A. Under the direction of the Clinical Manager/Clinical Director, communicates necessary information to field staff, physicians, patients, DME companies, referral sources, and community resources to facilitate the coordination and scheduling of patient care.

B. Receives, triages and fields telephone calls related to patient coordination and scheduling; receives and relays messages to appropriate personnel.

C. Receives staffing requests for clinical services for home patients from the Clinical Manager/Clinical Director. Assists in obtaining necessary information and scheduling appropriate personnel in order to staff cases according to patient care needs and availability of personnel.

D. Effectively communicates patient relevant information and staffing problems to appropriate clinician and supervisor.

E. Acts as a back-up to the Business Services Manager/Specialist in maintaining staffing records such as schedules for patient and staffing assignments and personnel files to assure compliance with regulatory compliance.

F. Under the direction of the Clinical Manager/Clinical Director, classifies, sorts and files records and department reference materials.

G. Under the direction of the Clinical Manager/Clinical Director, assists in the review, tracking, and trending of documentation focus areas or issues. Tracks timely submission of paperwork and completion of required supplemental orders.

H. Under the direction of the Clinical Manager/Clinical Director, completes billing audits to assure required documentation for billing is complete.

I. Under the direction of the Clinical Manager/Clinical Director, provides administrative support to the supervisor including assisting in new employee sign-up and orientation, scheduling clinician appointments with the supervisor, copying records or files, preparing information and field charts for clinicians, and other administrative support duties.
