UnitedHealthcare Community & State is currently hiring a Care Coordinator / Case Manager in the Coffeyville, Parsons, and Independence , Kansas area. The Care Coordinator / Case Manager will be responsible for, but not limited to the following duties:
-Engaging members telephonically to coordinate services, community resources, and treatment needs
-Delivering a holistic approach to coordinated care based on the enrollee needs
-Identifying early risk factors and conducting ongoing assessments and documenting in an electronic file
-Creating, reviewing and revising Plan of Care plans and focusing on disease management
-Functioning as an advocate for members and decreasing unnecessary hospitalizations
-Collaborating with the member's PCP to deliver and coordinate necessary services
-Building relationships with members and their families; assisting them with proper health choices
-Ensuring cases are documented in a timely manner

-Bachelor's level degree in Social Work or RN degree
-Minimum of 2 year experience in long-term care, home health, hospice, public health, or assisted living
-Basic level of proficiency in PC-based word processing and database documentation (Word, Excel, Internet, Outlook)
Additional Assets Preferred:
-A background working in geriatric special needs
-A background in behavioral health
-Experience with electronic charting
-Ability to multitask
