Evaluates patients in a homecare setting, plans and implements an effective Physical Therapy program based upon physical assessment, clinical judgement, and psychological sensitivity for patients with diverse diagnoses; provides patient education and consultation; supervises and delegates patient care to Physical Therapist Assistants (PTA's), Rehab Technicians, and Physical Therapy Students.
Required: Bachelors Degree
Preferred: Masters Degree
Required: Valid Illinois State Drivers License,CPR (Cardiopulmonary Resuscitation),Licensed Physical Therapist
Position Responsibilities: PATIENT ASSESSMENT/TREATMENT: Performs patient evaluations and treatments in alignment with the Physical Therapy Practice act and departmental policies and procedures to assess patients functional deficits and identify recommendations for intervention utilizing basic skills with occasional supervision throughout the entire span of life with minimal supervision. COMMUNICATION: Exhibits effective patient management skills to enhance comprehensive patient care and continuity. PROGRAM & STAFF DEVELOPMENT: Supervises and delegates appropriate patient care responsibilities to PTA?s, ATC?s, Rehab Technicians in adherence with PT Practice Act to ensure desired outcomes and patient safety. DOCUMENTATION: Performs required documentation for evaluation and treatment findings and communicates this in a competent, complete, accurate, clear, concise and timely manner to meet patient and/or family needs, legal and regulatory requirements. SERVICE EXCELLENCE: Assures customer satisfaction by exhibiting service excellence skills. CLINICAL INSTRUCTION: Supervises PT students by providing clinical experiences for affiliated schools.
