DUTIES: Back to top · The primary purpose of this position is to provide comprehensive professional clinical and preventive nursing services to assigned beneficiary population. Duty resides within a military ambulatory/primary care system (family health, pediatrics, and/or flight medicine) that emphasizes internal medical readiness, combat readiness of active duty/reserve population, and community/population health.
· Provide comprehensive Disease management Services, applying and promoting disease management, health promotion, and wellness concepts and strategies to assigned patients and those identified for disease management
· Provide highly specialized comprehensive primary, secondary, and tertiary prevention/intervention nursing services of an advanced nature and considerable difficulty to active duty/reserve/retired military members and their families
· Identify candidates for disease management using the Military Healthcare System Population Health Portal, Integrated Clinical Database, Complex Patient Management Tool, and professional judgment in collaboration with the Primary Care Manger and entire healthcare team
· Orient, train, and mentor civilian, military, and/or contract personnel which may include: medical/nursing assistants, administrative medical personnel, registered and/or licensed practical/vocational nurses, volunteers, and/or students, as assigned; participate in continuing education programs to update and/or maintain skills and knowledge to meet annual requirements.
QUALIFICATIONS REQUIRED: Back to top SPECIALIZED EXPERIENCE: At least one (1) year of specialized experience at the next lower grade (GS-09) or equivalent. S pecialized experience applying professional knowledge of a wide range of advance concepts, principles, theories, and practices to perform highly specialized nursing assignments of advanced nature and considerable difficulty requiring extended specialized training and experience; managing care principles, military and civilian population healthcare, case management, utilization management, military medical readiness and demand management functions; ability to case manage patient care needs, develop nursing care plans, manage and be a customer advocate continuously assessing quality improvement and outcomes measurements/management; knowledge of a wide range of medical disorders such as oncology, neurology, cardiovascular, thoracic surgery, general surgery, orthopedic, ophthalmology, urology, neurosurgical, gynecological surgery complications and indicated therapeutic intervention required; knowledge of patient-family health education/counseling principles and available resources to effectively apply them in patient interactions.

Federal Time-In-Grade (TIG) Requirement for General Schedule (GS) Positions: Applicants must have served at least 52 weeks at the GS-09 or higher grade (or equivalent in an alternate pay system) within the Federal Civil Service to be considered for referral. The purpose of TIG restrictions is to prevent excessively rapid promotions in competitive service GS positions and to protect competitive principles.

KNOWLEDGE, SKILLS AND ABILITIES (KSAs): Your qualifications will be evaluated on the basis of your level of knowledge, skills, abilities and/or competencies in the following areas:

1. Knowledge of an extensive range of advanced professional nursing care theories, principles, practices, and procedures to perform highly specialized disease management nursing care for patients.

2. Knowledge of a variety of pharmacological agents used in patient treatment, the desired effects, side effects, and complications of their use.

3. Knowledge of the administrative requirements for proper documentation of patients’ conditions including disease progress, acknowledgement of teaching, and follow up care.

4. Knowledge of the application of federal/state laws, professional principles/standards, and/or requirements related to: privacy, customer service, patient and family advocacy, staff and patient safety, infection control security of resources, documentation of nursing care, continuous quality improvement and outcome management. At all times, acts in accordance with DOD, AF, and MTF policies and directives.

5. Ability to provide guidance to other professional nurses and paraprofessional personnel.

6. Ability to communicate orally and in writing with a diverse group of individuals, to include use of the organization’s designated computer information systems and database programs and perform computer skills necessary to analyze, track, and quantify program effectiveness, and to manage the dissemination of information critical to the success of the managed care platform.

FOREIGN EDUCATION: Education completed in foreign colleges or universities may be used to meet the requirements. You must show proof the education credentials have been deemed to be at least equivalent to that gained in conventional U.S. education program. It is your responsibility to provide such evidence when applying.

ADDITIONAL CONDITIONS OF EMPLOYMENT :

1. Degree or diploma from a professional nursing program approved by the legally designated State accrediting agency at the time the program was completed by the applicant is required.

2. Must maintain a current Register Nurse (RN) license from a state or territory of the United States or District of Columbia.

3. Must maintain competency in Basic Life Support (BLS) certification. Certification in Advanced Cardiac Life Support (ACLS), Pediatric Advanced Life Support (PALS), and/or Neonatal Resuscitation Program (NRP) is highly recommended.

4. Certification as an Adult Clinical Nurse Specialist (ACNS) or Pediatric Clinical Nurse Specialist (PCNS) by the American Nurses Credentialing Center is highly desired.

5. Requires long periods of standing, sitting, and walking. May also require bending, reaching, and moving patients.

6. May be required to work rotating shifts to include evenings, weekends, and holidays.

7. Position is designated as a testing designated position (TDP) under the Air Force Civilian Drug Testing Program. Incumbent(s) of this position is subject to the possibility of a random drug test.

8. Position may be identified as having “regular contact with children”, requiring a State Criminal History Repository (SCHR) check to be completed on any person selected for the position.

9. Position may be designated mission essential based on local conditions. Mission Essential personnel report for duty at normal times when the base is closed or under delayed reporting, and remain on duty during Phased or Early Release. Supervisors make these designations based on mission requirements.

10. Complete all training requirements designated by facility, supervisor, or accrediting agencies; p articipates in continuing education programs to update and/or maintain skills and knowledge to meet licensing requirements.

11. For this nursing specialty, the following educational background and work experience are highly recommended: Baccalaureate of Science degree in nursing (BSN) from an accredited educational institution recognized by the National League of Nursing (NLN) or Commission on Collegiate Nursing Education (CCNE); minimum of five years of experience as registered nurse with at least 12 months of experience in specific or related clinical area. Masters Degree in nursing, managed care or other health-related field is highly desirable. National nursing certification in chronic care (i.e., Chronic Care Professional certification) or a related field (e.g., medical-surgical, pediatrics, ambulatory care, nursing administration, etc.), is highly recommended.

12. May be assigned to MTF disaster team; may be required to participate in applicable team training/education and exercises during normal duty hours. In event of an actual disaster, may be recalled and required to respond to team assignment.
