The Patient Care Coordinator leads and directs the local home care team. This position is responsible for ensuring the delivery of quality patient care. The Patient Care Coordinator is also focused on internal and external customer service that exceeds the expectations of referral sources, patients and their families. The Patient Care Coordinator manages the day-to-day operations of the home care team. This position promotes and maintains the image of the agency in the area served according to agency mission, philosophy and all regulatory standards.
Registered Nurse with a current nursing license, BSN preferred. Ability to apply for TN license within 30 days of hire. Minimum three years nursing practice with at least one year in a hospice setting. Previous management experience is desired.
