LPN will serve as .5 FTE Women's Health and .5 FTE Primary Care focusing on clinical visits, exams and treatments with provider. The individual will be responsible for observing, recording and reporting changes in the patient's condition, as well as assisting in the development and revision of the patient-driven holistic care plan for life. Will work collaboratively with Core Team (Patient, Primary Care Provider, RN Care Manager and Patient Support Technician ) and Expanded Team (family/caregiver, internal and community based services involved in providing care to the patient). Facility has implementation of Patient Aligned Care Teams (PACT) in areas consistent with Nursing Care Management and Coordination of Veterans. Clinical hours are Monday-Friday, 8-4:30.
The full performance level of this vacancy is GS-6. The actual grade at which an applicant may be selected for this vacancy is in the range of GS-4 through GS-6. The Professional Standards Board (a peer-review group) will review the selected candidate's education and experience and recommend the grade and step at which the candidate will be placed. The salary is based on the grade and step approved for the selected candidate.

Graduate of a school of practical or vocational nurse school
approved by the appropriate State agency and/or accredited by NLNAC.

GS-5 one year of qualifying experience at the GS-4 level or equivalent, fully meeting all performance requirements for the GS-4 LPN/LVN.
GS-6 one year of qualifying experience at the GS-5 level or equivalent, fully meeting all performance requirements for the GS-5 LPN/LVN.
