The Clinical Coordinator directs, supervises, evaluates, and coordinates patient care services provided by department personnel to insure the delivery of quality patient care. Determines staffing needs and case assignments in collaboration with Team Leaders on a daily basis. Facilitates Team Conferences. Assists with ongoing Quality Assessment and Improvement, Utilization Review, and monthly billing. Encourages a harmonious, high-performance work environment characterized by positive relationships and a strong team orientation while providing the maximum amount of autonomy possible for staff members. Hires, trains, supervises clinical employees and interprets personnel practices; assists in preparation of annual budget, meets licensing and other authorized inspection groups; promotes favorable public relations and interprets the services of the facility to news media and the public; assists in the processing of admissions and coordinates work with state social and health agencies. Assists with utilization review and monthly billing activities.
Reviews patient records for accuracy, eligibility, and to assure all documentation is current and complete by auditing admission charts.
Assures patient record documentation is timely, correct, and within federal, state, and department guidelines by auditing open and closed records, notes number of late recerts, lock dates and discharges.
Assures quality care is delivered as prescribed by the attending physician, regulatory guidelines, and department policies and procedures.
Reviews “call sheets” daily.
Reviews ER visits and hospital admissions daily.
Assists with financial management to insure cost-effective patient care, and that supplies are used in a cost-effective manner to avoid waste.
At billing, reviews number of visits written off and number of orders written to cover services rendered. Monitors billing error trends then assists with educational training, coaching, counseling of staff.
Handles counseling and disciplinary processes, following department policy and procedures, after consultation with Clinical Services Manager as indicated.
Assures staff work within their job descriptions, maintaining standards of care within regulatory guidelines.
Assures patient assessment and care are within appropriate standards and as reflected on the Plan of Care.
Assures patient assignments are based on patient and department needs, nursing skills, and case load levels.
Participates in evaluation of orientation of new personnel to department, including policies and procedures.
Evaluates and assesses educational/clinical needs and performance of staff members to provide instruction for new policies, procedures, equipment, or regulations.
Participates in instructing staff on all aspects of reimbursement and regulatory guidelines and assures adherence.
Assures timeliness of team conferences and staff meetings.
Graduate of an accredited school of nursing required.
Bachelor of Science degree preferred.
Prefer at least two (2) years experience in nursing in long term care. Previous supervisory experience preferred.
Is a graduate of an accredited school of nursing and is currently licensed as a registered nurse in the State of Wisconsin. BSN preferred.
