To work collaboratively with all members of the multidisciplinary team to assess patients/families, provide interventions and facilitate discharge for patients with complex psychosocial needs.
Provides services addressing the needs of patients/families with and emphasis on psychosocial functioning, coping and adjustment to illness, disability and loss.
Manage complex psychosocial issues such as, abuse, unstable living environment, substance abuse, family conflict, decision making and end of life issues and facilitate adoptions/surrender newborns to facilitate patient flow and discharge planning/placements to support the continuum of care.
Requirements: JACHO-State of
Social Work License Required – Masters in Social Work
Experience in Medical Social Work and discharge planning preferred along with experience in individual, family and group assessment, psycho-therapy, crisis intervention, grief and loss counseling, resource networking.
Each social worker will become clinically proficient in a specialty area (Ortho), but is also prepared to cover other areas.
