La Crosse County Health Department seeks professional to provide nutrition services to Women's, Infants' and Children's (WIC) Program participants, provide follow up to families enrolled in FIT Families Program, case manage high risk pregnant women enrolled in the prenatal care coordination program and provide nutrition education to employees and the general population.

Hours of Work: This position will work 30 hours per week Monday-Thursday between the hours of 8:00am and 4:30pm. Although this is not a full-time work schedule, the insurance benefits offered for this position are equivalent to that of a full-time position.

Essential Job Functions:
For a complete job description, see the job descriptions tab of our website. Develop, plan and conduct WIC secondary nutrition education. Select, develop, evaluate and update nutrition education materials used by the La Crosse County WIC Project. Provide consultation to nurses, physicians and other health professionals. Assess nutritional status of WIC participants, provide counseling and follow up, select appropriate WIC food package, make appropriate referrals, participate in participant-related team conferences, make home visits in selected cases and document care in participant files. Provide high risk and follow up nutrition care. Serve as a nutrition resource to other health department staff and the County Occupational Health Program. Conduct weight management programs for interested La Crosse area residents. Provide nutritional support to patients on home care/CMO caseload. Develop and conduct healthy living programs on topics of interest to the community. Assess potentially eligible pregnant women for care coordination services and enroll those found eligible. Assist women enrolled in the prenatal care coordination program gain access to the necessary medical, social and educational services they need to enhance their pregnancy outcome. Provide follow-up services for 60 days post partum to all women enrolled in the Prenatal Care Coordination Program. Identify and enroll interested families with preschool children in the Fit Families program. Collect required baseline and post intervention data, work with family to set a fit families’ goal the family will work on during their tenure on the program and work with the family to identify strategies they are willing to use to achieve their goal. Contact the enrolled family monthly to follow up on their progress achieving their goal. Document all contacts with the family in the Fit Families tracking form. Work with other Fit Families staff to create a clinic and community environment that supports healthy eating habits an active lifestyle.

Training, Experience and Other Requirements:
  • Graduation from an accredited college or university with a Bachelor of Science Degree in Foods/Nutrition, Nutrition Education, Dietetics, or other suitably related field.
  • Preference for one year of professional nutrition educator experience.
  • May be required to meet the requirements for professional registration or be a Registered Dietitian.
  • Valid Wisconsin driver's license and access to vehicle, or other reliable means of transportation for city and rural travel.
