Provides active treatment and skills training according to each Individual Program Plan (IPP) to assist Individuals to function independently in self-help &/or daily-living skills (e.g. eating, bathing, toileting, personal hygiene, oral hygiene, dressing, grooming, communication, money management); trains the individuals to serve themselves & assist in pre & post meal set up; assures access to personal possessions & assures individuals are dressed appropriately; accompanies and trains individuals in programs at off- living area program areas as described in the individuals’ IPP; assists with planning & implementing therapeutic leisure-time activities including community trips (e.g. shopping, movies, out to eat, hair cut appointments, grocery shopping, banking); institutes appropriate behavioral intervention for unacceptable behavior as defined by approved behavioral plans; completes required daily documentation; provides level of supervision as determined by Individuals’ IPP; maintains a positive learning environment by providing positive reinforcement, encouragement and promoting an atmosphere of choice and self management. Communicates with the individual’s in a manner indicative of a relaxed atmosphere and interacts with them in a positive, constructive manner. Mandatory overtime is required of this position; participates as a member of the Interdisciplinary Program Plan Team in the development, implementation, evaluation & modification of individual’s Individual Program Plans (IPP’s); attends team meetings/staffings (Annuals and Specials) for assigned individual's, serving as direct care representative; recommends changes in individual program plans as behavior shows progress or regression; completes monthly progress summaries and yearly assessments and clothing inventories; observes for and reports reaction to medication & any unusual incidents; assists nursing by obtaining vital signs as necessary, completes treatments (e.g. applies ointments); assists in keeping living environment safe; shops with individual's for clothing &/or personal belongings; completes Incident Reports as necessary. Operates a motor vehicle to transport individuals to on-grounds & off-grounds activities (e.g., transports individuals to educational, recreational or occupational activities, shop for personal items, medical appointments including local and out-of-town locations [e.g. Ohio State Medical Center Hospitals, Cleveland Clinic] , workshops transports individual's to & from appointments & classes on & off grounds.) Operates equipment necessary to teach individual's activities prescribed on individual treatment plans (e.g., washer, dryer, dishwasher, sweeper, kitchen appliances); assists in the maintenance of a clean & sanitary living environment by assisting & teaching individual's how to mop floors, wash windows, dust furniture, complete personal laundry; performs other related duties (e.g., mentors new employees, attends in-services [e.g., first aid, fire safety, CPR, PACES]).Attends meetings and in-service programs as applicable; keeps self updated on Center’s policies and procedures; completes all other related duties as assigned.
Formal education in arithmetic that includes addition & subtraction & in reading, writing & speaking common English vocabulary.
For positions that require operation of motor vehicle to transport consumers to recreational, educational &/or occupational activities, a valid driver’s license is required.

Major Worker Characteristics:
Knowledge of J.C.A.H. (i.e., Joint Council for Accreditation of Hospitals) standards for resident care or I.C.F.M.R. (i.e., Intermediate Care for Mentally Retarded) standards for resident care & Resident Review & Licensing Regulations*. Skill in operation of motor vehicle; direct care procedures (e.g., taking of vital signs, rendering of tub baths, appropriate lifting & positioning procedures, first aid). Ability to deal with problems involving several variables within familiar context; maintain accurate records; cooperate with co-workers; develop good rapport with residents &/or patients; demonstrate physical strength to lift 50-100 lbs.; read short sentences with concrete vocabulary.
