Social Worker / MSW / Hospice / Palliative Care The Social Services Coordinator is an integral member of the home health interdisciplinary team by providing direct counseling services to patients and families. Coordination of intervention design, facilitation of grief group, participation in education programs and consultation with community groups and professionals.       Essential Job Functions:
      • Participate in regular scheduled interdisciplinary team meetings and individual conferences, as needed, to facilitate coordination of care.
      • Contribute towards identifying social problems, their severity, inter-relatedness to the medical situation, in addition to assessing family strengths and weaknesses with focus upon methods to alleviate problems.
      • Emotional support to assist with the anxiety and stress precipitated by illness, loss, disability, and death which will work to reduce the patient's and families emotional discomfort and facilitate adjustment to present life situation.
      • Will report to and update the interdisciplinary team on bereavement services.
      • Maintains appropriate clinical records and reports as required.
      • Develops and presents specific bereavement training for staff and volunteers as needed.
      • Establishes a support group, if indicated, for families according to established policy.
      • Arranges for home visits of clergy and other religious organizations in the community as needed.
      • Provides other necessary counseling services (such as financial) and makes referrals to community resources as needed.
      • Completes a plan of treatment on patients that require medical social services.
      • Participate in Performance Improvement program and assist with PI data retrieval when requested.
      • Is available on request to present community education programs to churches and other organizations.
      • Maintains confidentiality in respect to all home health matters.
      • Must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned department by assessing, treating or caring for all patients physiological, psychosocial, and/or learning needs to determine priority of implementation of age appropriate social services interventions and teaching plan based upon the discipline represented. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to the patients age specific needs and to provide the care needed as described in the department's policies and procedures. Age specific categories include pediatrics, adolescents, adults, and geriatrics.

      • Keep abreast of current professional standards in the filed of home health and social work and makes recommendations in terms of policy, procedures and programs.

      Qualifications
      Social Worker / MSW / Hospice / Palliative Care
      • Master's Degree in Social work. If not a MSW must have an MSW sign off on visits.
      • Good Communication skills.
      • Must have current driver's license, automobile and auto liability insurance which meets the Massachusetts financial responsibility laws requirements.
      • Interest/experience with Home Health patients and families.
      • Strong background in counseling.
        Must be a Licensed Social Worker with the State of Massachusetts. Social Worker / MSW / Hospice / Palliative Care Physical Requirements: Visual/hearing ability sufficient to comprehend written/verbal communication. Ability to perform tasks involving physical activity (In good physical health). Ability to drive an automobile. Ability to deal effectively with stress.
