The Pre -Transplant Coordinator (PTC) is responsible for coordinating patient care prior to transplant; from the time of initial referral until the time a transplant is performed. The PTC conducts the initial pre-evaluation patient interview and education; schedules studies or procedures for diagnostic purposes; prepares the patient chart for the transplant evaluation committee meetings; coordinates services with the local organ procurement organization and provides education to internal and external professionals. The PTC assists patients through the evaluation process by working closely with the local physician and dialysis unit. The PTC maintains regular contact with the patient and local dialysis unit and/or physician, monitoring intercurrent medical events to ensure the readiness of each patient for transplant. The PTC has no post transplant responsibilities.
Minimum Knowledge, Skills, and Abilities Required
Bachelor's degree preferred; RN, PA, NP, recent critical care or other related experience.
Maintain licensure and certification of professional stature.
Strong verbal, written and interpersonal skills.
Ability to work independently, in group situations and with the public.
Ability to organize and prioritize work to meet deadlines.
Ability to work under stress with numerous interruptions, distractions and changing priorities.
This is a full time position, and requires on call coverage. Critical thinking, case management skills, renal, and/or critical care experience preferred.
