PURPOSE: The purpose of this position is to facilitate project and operational improvement needs, working with IT and/or other departments, where efficiency improvements have been identified. Manage all aspects of projects.

ESSENTIAL FUNCTIONS:
Analyze data gathered and develop solutions or alternative methods of proceeding.
Review forms and reports, and confer with management and users about form, distribution, and purpose, and to identify problems and improvements.
Identify underlying principles, reasons, or facts of information by breaking down information or data into separate parts.
Create and update flowcharts as needed per request.
Translate or explain what information means and how it can be used.
Provide information to supervisors, co-workers, and subordinates by telephone, in written form, email, or in person.
Perform system analysis to determine how system(s) should work and how changes in conditions, operations, and environments will affect outcome(s).
Facilitate design meetings and any additional meetings necessary to complete project in a timely manner.
Create Project Impact Reports to document project delays with the necessary reasons.
Set deadlines, assign responsibilities, and monitor and summarize progress of project or audit.
Prepare reports for upper management regarding status of project, assignment, and/or audit as needed.
Update Project Status Log weekly and any other assigned logs and/or databases to ensure up to date communication of information.

WORK EXPERIENCE:
Minimum of one year experience in a home health environment and at least 6 months in a project specific position.

EDUCATION:
Bachelor’s degree in Business Management or Information Systems preferred.
Equivalent combination of education and experience.

KNOWLEDGE, SKILLS, ABILITIES and OTHER CHARACTERISTICS:
Knowledge of PMO standard practices preferred
Knowledge of principles and processes for providing customer and personal services. This includes customer needs assessment, meeting quality standards for service, and evaluation of customer satisfaction.
Must have computer skills.
Must be proficient in Microsoft Office (Word, Excel, PowerPoint & Outlook).
Strong analytical and problem solving skills.
Good attention to detail a must.
Strong interpersonal skills essential.
Ability to read and understand information and ideas presented in writing.
Ability to communicate effectively, both verbally and in writing, with individuals inside and outside the Company.
Exhibits a professional manner in dealing with others and works to maintain constructive working relationships.
Exhibit good problem solving abilities and demonstrate the ability to work well under deadline driven environments.
Should be self-motivated.
