ACLD is a leading not for profit agency on Long Island who for over fifty years has served the needs of nearly 3,000 children, teens and adults with developmental disabilities and their families. Our mission is to provide opportunities for individuals with disabilities to pursue an enviable life; to increase their independence and improve the quality of their lives in the community. Over 1,000 full and part-time staff are employed across an array of more than 70 program sites in Nassau and Suffolk Counties. ACLD operated 28 community-based residences and 37 apartment sites in two counties, serving 320 adults in residential placements. These residential programs provide counseling, training, and support services to enable individuals with disabilities to work, live and recreate in the mainstream of community life on Long island. Registered Nurse The role of Registered Nurse Facilitator in our Residential Program is to assist the Nursing Administrators in ensuring that all medical and nursing services are provided to consumers as per OPWDD regulations currently in effect. This position will assist in the oversight of multiple group homes located throughout Nassau and Suffolk counties. Essential Functions Complete all mandatory medical assessments within specified time frames, to include: Annual Self-Medication Evaluation Review Nutritional Assessments and address needs in a timely manner Meet with consulting pharmacist to assist in pharmacological reviews, as applicable, and address recommendations made in a timely manner. Ensure that all medical appointments/requirements are scheduled and followed-up, as indicated. All diagnostic tests and laboratory results are to be obtained in a timely manner and reviewed/signed off on, with any physician review/notification necessary. Assist, when necessary, in accompanying consumers to medical appointments Review and sign off on all medical visit forms, and update the Record of Medical Appointments (“Flow Sheet”). Report changes in consumer’s health status to the physician, residence staff, inter-disciplinary team, day program nursing staff, RN Administrator, and consumer’s parents, as applicable. In addition, day program nursing staff is to receive copies of all medical reports, and prescriptions. Clinic should receive non clinic reports. Document any significant medical/behavioral information in Nursing Progress Notes Ensure proper maintenance of all medical records, medications, medical/ first aid supplies and personal protective equipment. Maintain the Medication Book, to include on-going review of the Medication Administration Record, Physician orders, Resident Specific Medication Information Records, etc. Ensure the maintenance of monthly weights and vital signs. Ensure maintenance of daily bowel movement/incontinence charting, as applicable. Obtain all necessary informed consents for medical procedures, as applicable; and maintain all Psychotropic Informed Consents (ICF only). Provide all staff and consumers with required training. Provide on-going supervision and follow-up training of AMAP staff. Assist with AMAP certification process, to include AMAP pourings, and vital signs testing. During consumer’s hospitalization, act as a liaison between ACLD’s residential department and the hospital. Coordinate care, facilitate discharge planning, as assigned provide on site hospital visits Provide on call coverage to assigned houses from 9am Monday until 5pm Friday and adheres to the agency’s Telephone Triage Protocol. Takes a proactive/preventive approach to healthcare (i.e., long term healthcare planning, seeks out alternative and/or second opinions, includes clinicians when a change from baseline appears), and is knowledgeable regarding the medical issues at assigned residence. Follows incident-reporting procedures consistently (fully investigates injuries, ensures follow up and preventative actions). Attends and participates in OMRDD/Self Surveys, and the POCA response Conducts and thoroughly documents weekly Medical Coordination meetings with the Residential Manager. Participate in ongoing trainings and/or conference to ensure updated professional practice with all consumers. Participate in clinical meetings, case conferences, annuals/semi-annuals, as applicable; and complete required reports in a timely manner. Attend monthly residential nursing meetings and required in-services Participate in weekend emergency on call beeper rotation, as applicable Other duties as assigned Please apply on-line.

Job Requirements :
Requirements RN certification, currently licensed by the New York State Department of Education; including all mandated certifications. General computer knowledge preferred. The ability to lift approximately 10 pounds. General knowledge of medications and their interactions. Two years RN experience, one year MRDD experience strongly preferred. CPR certification Additional Knowledge, Skills and Ability Excellent verbal and written communication skills including the ability to coach, aid council, interpret and advise, and the ability to professionally develop and report problems. Ability to work independently. Ability to recognize and respond to emergency situations. Ability to prioritize when handling multiple tasks.
